FORM

 [Local Convention]
The FORM element is used to delimit a data entry form . There can be several forms in a single document, but the FORM element can't be nested.

It is important to understand that the use of FORM requires that someone at the server end of your operation has done some programming to process the form. Including a FORM in your document without arranging for the necessary scripts to be written and installed, is likely to leave your reader with an error message.

FORM may not be used simply to effect a typographic result
