slipcases

Some friends and I have copies of the (trade edition of) Barry Moser's
bible, which measures about 8-5/8 x 12-1/4 x 2-1/8. The friends want
slipcases for their copies.

Do most bookbinders/conservators do this type of work? Is it always custom
work? Is it necessary to have the book in hand to be able to create the
slipcase?
