I just recently discovered Wormhole Books (www.wormholebooks.com) which
prints chapbooks. They say:

 Wormhole Contemporary Chapbooks are a series of collectible short fiction
 by today's most original authors in the science fiction, fantasy, horror,
 and mystery fields. Each chapbook is a miniature work of art, printed on
 premium acid-free papers, individually-numbered, and limited to a
 maximum of 750 numbered and 52 lettered copies, as well as 200 numbered
 hardbacks. Most are first editions and cannot be found elsewhere.

Apparently they do as much of their work in-house and by hand as possible.
